Nomination Instructions

Please paste your nomination letter in the space provided on the form. Be sure to keep a copy for your records. Your nomination letter should speak specifically to the criteria of this award.

Please note: Nomination letters should not exceed 600 words.

All nominations should include the following:

• A description of how long you have known the nominee and in what capacity
• A list/description of the nominee's contributions and achievements (addressing both the breadth and depth of each contribution or achievement)
• A description (with specific examples) of how the nominee's leadership embodies and demonstrates the ideals indicated in this particular award's criteria

We recommend that nominators address the following (if applicable):

• Leadership is about involving and influencing others. How did this nominee involve others?
• Please address the intent of the nominee's contributions.
• Did the nominee overcome adversity or challenges? If so, how?
